  IRS Form 8655 Reporting Agent Authorization for Electronic Filers
  IRS Form SS4 Employer ID Request
  IRS Form 8821 Tax Information Request Authorization

  CA Form DE48 Power of Attorney
  CA Form DE1 State ID Request

Fill out each form completely and sign it. Then fax the completed forms to Accuchex at 1-415-883-7080.

Make copies of the forms for yourself and then send the signed originals to us:
